This pathway is for people who may need AAC, daily living support, caregiver assistance, medical monitoring, safety planning, low-demand communication, and multi-agency coordination.

Communication-first

Respect AAC, gestures, objects, typed text, trusted supporters, delayed processing, and non-speaking states.

Medical and daily living

Track routines, medication, hydration, sleep, pain, seizures, GI, BP/O2, and caregiver instructions in a stable format.

Safety without escalation

Reduce demands, check medical causes, protect dignity, and avoid interpreting distress as intentional noncompliance.

Caregiver transfer

Build a backup binder so one caregiver is not the only memory system for care routines and emergency information.
